DO YOU QUALIFY?
• Must have diabetes mellitus
• Tests their blood sugar 4 or moretimes per day
• Treated with 3 or more daily injections of insulin or is on insulin pump
• Has to have in-person visit within 6 months of CGM order
• In-person visit must be scheduled every 6 months after initial visit
